Requirement to employ housemaid:

Passport size photo for Husband & Wife (each 1 pc)

Latest Borang B for Husband & Wife (must be exceed RM36,000.00)

If no From J available, documentation should be needed:
- Photocopy of E.A. Form
- Company original employment letter
- Latest original three (3) month’s salary slip
- Latest original E.P.F. statement

Applying maid to look after patient or old folks, document needed as below:
- Photocopy of employer birth certificate
- Photocopy of patient/old folks I/C
- Original medical report of patient / old folks certified by specialist doctor.
- 1 pic photo of the patient / old folks
Apply for maid monthly conditions:
- PHILIPPINE - RM5,000.00 / month (must be exceed RM60,000.00)
- INDONESIA - RM3,000.00 / month (must be exceed RM 36,000.00)
- VIETNAM - RM3,000.00 / month (must be exceed RM 36,000.00)
- CAMBODIA - RM3,000.00 / month (must be exceed RM 36,000.00)
- SRI LANKA - RM5,000.00 / month (must be exceed RM60,000.00)
申请女佣文件如下：

夫妻身份证复本。（必须清楚）

结婚证书复本。

夫妇彩色照片一人各两张。

孩子报生纸（必须十五岁以下）。

夫妇最新所得税复本（年收入至少RM36,000.00 /RM 60,000.00）&公司或个人最新三个月银行来往户口复本。

若无所得税，必须上以下文件：
- 最新EA FORM复本
- 公司聘请信复本
- 最近三个月的薪水单复本
- 最新公积金复本

照顾老人所需文件：
- 申请人报生纸复本
- 病患者之身份证复本
- 病患者之医药报告正本
申请女佣的月入条件：
- 菲利宾 - RM5,000.00 / 月（年收入RM60,000或以上）
- 印尼 - RM3,000.00 / 月（年收入RM36,000.00或以上）
- 越南 - RM3,000.00 / 月（年收入RM36,000.00或以上）
- 柬埔寨 - RM3,000.00 / 月（年收入RM36,000.00或以上）
- 斯里蘭卡 - RM5,000.00 / 月（年收入RM60,000或以上）
